2. Services Description

Skinny Science provides medically guided weight loss services via telehealth throughout the state of Arizona. Our services include but are not limited to:

• Smart & Lean Blueprint weight loss program
• MicroMethod Program for metabolic balance
• GLP-1 medication management
• Telehealth consultations and follow-up appointments

3. Patient Eligibility

Our services are available to adults aged 18 and older who reside in the state of Arizona. By using our services, you represent that you are at least 18 years of age and an Arizona resident. Certain treatments may have additional eligibility requirements based on medical history and clinical assessment.

4. Payment Terms

Skinny Science operates as a cash-pay practice. We do not bill insurance directly. We accept major credit/debit cards, HSA (Health Savings Account), and FSA (Flexible Spending Account) payments. All fees are due at the time of service. Program pricing is outlined during your consultation. Refund policies will be discussed at the time of enrollment.

5. Telehealth Consent

By using our telehealth services, you consent to receiving healthcare services remotely via audio and/or video technology. You understand that telehealth is not a substitute for emergency care. In case of a medical emergency, you should call 911 or go to the nearest emergency room. You acknowledge that technical difficulties may occasionally interfere with telehealth sessions and agree to contact our office to reschedule if needed.
